2011 Victory 100 Volunteer Recognition Award

Victory 100 Award recognizes the dedicated volunteers for their many hours of hard work and support they give UCP through affiliates across the country.

Please submit the names of your local volunteers who have a minimum of 100 hours of service during 2010. The period of service to be considered should be January 1 - December 31, 2010. At the time of nomination, the affiliate must be in good standing with the national organization. Those names submitted will be honored at the Awards for Excellence Dinner during the 2011 United Cerebral Palsy Annual Conference and will receive a certificate of appreciation as a Victory 100 volunteer.

Nomination Deadline:

 Forms and supporting documentation must be received at the National Office COB on Friday, February 11, 2011.  There will be no extensions.

 **Please ensure that the person or organization being nominated is not subject to any action that might cause awkwardness for UCP and its affiliates.
