Financial Assistance
To qualify for a grant, the applicant must be a resident of Stearns, Benton, or Sherburne county and be diagnosed as having cerebral palsy. Your request will be reviewed by our Financial Assistance Committee (FAC) during their next meeting on the first Wednesday of each month. The committee will not approve requests for equipment that has already been purchased (this includes credit card purchases). Each request is reviewed on an individual basis. Requests may be granted with full payments or partial payment. Many factors are included in the decision-making process and it also depends on what funds we have available at the time of the request.

If the FAC feels they need additional information before making a decision, you will be contacted. Please try to answer all of the questions to avoid delays in the process. Please include a picture or a photocopy of the equipment, estimates you may have received or a recommendation from a professional for this request.

If your request for funding is approved, U.C.P. will request that you volunteer some time to help us out during the year at our special events. Your volunteer help is always greatly appreciated. We also require a thank you note and a photo of the recipient. This is necessary for us to renew our funding from outside sources so that we can continue to fund other recipients.

You will be contacted as soon as a decision has been made. If you have any questions while completing this form, please call me.

Sincerely,
Judy K. Moening
Executive Director
