MCDAB Needs Assessment Survey Online
The Madison City Disability Advocacy Board (MCDAB) was established this summer to strengthen public understanding of the needs of persons with disabilities and to ensure that all citizens with disabilities have equal access to the City of Madison’s resources and opportunities.
The MCDAB needs your input now on a survey to determine the needs of persons with disabilities within our community.

As the parent of a child with a disability, you understand the additional challenges faced by those living with a disability. This is your opportunity to make a significant difference for residents of the City of Madison who have a disability. Your unique input on this survey will help this board to more effectively advocate for the necessary resources, including funding, to address identified gaps in service (current and those anticipated in the future) and other barriers to equal access to all of City of Madison’s resources and opportunities.

Individuals, parents, or caregivers may complete the survey. In the event there is more than one person in a household who qualifies as disabled, a separate survey for each person should be completed. This survey is intended to be completed only by residents of the City of Madison who have a disability as defined by Section 504 of the Rehabilitation Act of 1973.
