Eva Cook Jones Educator of the Year Award

Dear Friend:

United Cerebral Palsy of Central Minnesota is proud to announce the Eva Cook Jones Educator of The Year Award.

We realize the important role educators play in the lives of people with disabilities and we want to recognize that special person who has made a significant difference in the life of a student with special needs.

The purpose of this award is to recognize Eva Cook Jones, a person who has made a significant difference in the lives of many people with disabilities. Through her perseverance and dedication to students with special needs, many of them became independent and successful in their lives. Without her help and influence in their education, they would not have achieved their potential.

Please consider someone you know for this award. It should be someone who has made a difference in the life of a student with a disability. . . . someone who goes the “extra mile” to help students realize their potential.

We hope to raise awareness of the challenges of educators and recognize those who deserve this recognition. UCP will honor this person at our annual meeting this fall by awarding them with a certificate of appreciation and a $200 cash gift.

Enclosed is a nomination form. Please forward it on to anyone who may be interested, or complete it today and fax or mail it to UCP by June l, 2009. Please don’t set this aside and forget about it, help us give someone the recognition they deserve.
