Independence Waiver

The Independence Waiver provides in home, community-based services for daily living assistance and prevents admissions to nursing facilities.

Eligibility:

• Physical disability and functional limitations
• Nursing home level of care
• Over 18 years of age
• Medical Assistance eligible
• Not ventilator dependent
• Total services must be under the cap of the average yearly nursing home costs

  Services Provided:

  • Service Coordination
  • Attendant Care Services
  • Respite
  • Personal Emergency Response
  • Physical, Occupational, and Speech therapies
  • Visiting Nurse
  • Limited assistance with modifications to the home and equipment Counties Served: York, Adams, and Franklin
