Transition Program (Decatur)
Through the Transition Program, UCP assists with information, guidance, and education on issues of transition for adolescents with developmental disabilities and their families. In addition, "TeenAbilities" is an after-school transition program designed to help adolescents with developmental disabilities learn basic life skills to help them as they transition from school into the community.
Services can happen at UCP Building or out in Community based sites. Frequency of service depends on the needs of the individual being served. TeenAbilities meetings are held at the UCP building. Fieldtrips are scheduled throughout the community. TeenAbilities is offered once per month during the school year.
