Family Support / Respite

Case Management

The Family Support Administrator and the Respite Case Manager provide case management services to participants enrolled in the Home Based Support Services program (HBSS), and the Family Assistance program (FAP.) FAP and HBSS service facilitation includes regular, on-going contact with consumers. Regular, on-going contact is defined as at least once a month, or more as requested by the consumer. The state provides for up to three hours of case management services per month. If the need exceeds this amount, the consumer may purchase more using his/her funding from DHS. These programs are federally/state funded Home and Community Based Waiver Services.

Families not eligible for Home and Community Based Waiver Services may still be eligible for case management assistance in other ways. Family Support staff are willing to assist families with guardianship, applications for governmental programs, such as Medicaid or Supplemental Security Income, and other relevant program areas. Referrals may be made to other organizations, such as legal aid services, to assist with these areas as well.
